According to Rare Record  Price Guide, the 4th and 5th lacquers were made for the 2nd pressing in 1965.  This record is from the 2nd pressing.

The vinyl appears Near Mint with only a few soft scuffs, zero scratches and soft spindle markings to the label.  the album was treated well over the past 41years.  Sound is excellent with only soft surface noise between tracks.  There is great clarity of tone with excellent low frequency kick.  Mids and highs seem natural with none of the harshness I have associated with later Capitol pressings of this LP that I've heard.
